ABSTRACT

Abstract An adult male puma (Puma concolor), hit by a car in an urban area, died three days later despite the therapeutic support provided. At necropsy, multiple firm nodules were identified in the gastric mucosa. The nodules were coated by an intact mucosa with a central opening from which reddish and cylindrical nematodes protruded into the lumen. Twenty-seven nematodes were retrieved for morphological and morphometric evaluations. During histopathological examination of the gastric tissue, the adult nematodes appear in longitudinal and transverse sections, surrounded by thick bands of collagen, interspersed with mixed inflammatory infiltrates. The nematodes had an eosinophilic cuticle with caudal serrated projections (bulbar type), coelomyarian musculature, pseudocoelom, and females with uterus containing numerous larvated eggs, characteristics consistent with the Cylicospirura genus. Morphologically, female nematodes had six large tricuspid teeth in the oral cavity and the vulva had an opening anterior to the esophagus-intestinal junction. Male nematodes had five pairs of small papillae near the tip of the tail. These findings were consistent with Cylicospirura felineus. This parasite should be included in the differential diagnosis of nodular gastric wall lesions in wild felids.


Resumo Uma onça parda (Puma concolor) foi encontrada em uma área urbana após atropelamento e, apesar do suporte terapêutico fornecido, o animal morreu três dias depois. No exame post-mortem, múltiplos nódulos firmes foram identificados na mucosa gástrica. Os nódulos eram revestidos por mucosa intacta com um orifício central, do qual se insinuavam nematódeos cilíndricos e avermelhados. Vinte e sete nematódeos foram recuperados para avaliação morfológica e morfométrica. Na avaliação histopatológica do tecido gástrico, os nematódeos adultos apareceram em cortes longitudinais e transversais, circundados por bandas espessas de colágeno, intercaladas por infiltrado inflamatório misto. Os nematódeos eram constituídos por cutícula eosinofílica, com projeções serrilhadas voltadas caudalmente (do tipo bulbar), musculatura celomiariana, pseudoceloma e, nas fêmeas, útero com numerosos ovos larvados cujos achados foram sugestivos do gênero Cylicospirura. Morfologicamente, a cavidade bucal continha seis grandes dentes trífidos, na fêmea, a abertura da vulva era anterior à junção esôfago intestinal; e os machos tinham cinco pares de pequenas papilas próximas à ponta da cauda. Esses achados foram consistentes com Cylicospirura felineus. Este parasita deve ser incluído no diagnóstico diferencial de lesões nodulares da parede gástrica em felinos selvagens.

ABSTRACT

Helicobacter pylori commonly is associated with gastritis: but only sometimes it causes clinically significant diseases such as gastric and duodenal ulcer. The development of disease depends on the virulence of the infecting H. pylori strain, the susceptibility of the host, and environment co-factors. The cytotoxin associated protein encoded by cagA gene is an important virulence factor that is produced by some H. pylori strains, and has been used as virulence marker in some populations. The aim of the study was to examine the prevalence of cagA gene in the isolated strains of H. pylori from patients with dyspeptic disease and to investigate the association of cagA gene and the severity of H. pylori related diseases in Iran. In this study, biopsy specimens were obtained from the antrum of 180 patients. After isolation of H. pylori and its DNA by standard methods, polymerase chain reaction [PCR] technique was used for detection of cagA bacterial gene. 92 out of the 180 patients had H. pylori strains. 70% were cagA gene positive. All patients with peptic ulcer [100%] and 44 out of 72 [61%] patients with non-ulcer dyspepsia were cagA positive [p<0.01]. There was significant difference in frequency of cagA gene in peptic ulcer disease and non-ulcer dyspepsia [p<0.01]. It showed that the risk of PUD in patients with cagA+ H. pylori infection may be higher than in those with cagA- H. pylori infection

ABSTRACT

Objetivo: establecer relaciones entre los hallazgos histológicos y endoscópicos en la mucosa gástrica y la identificación de H. pylori en niños en que se realizó endoscopía digestiva por distintas razones. Pacientes y método: Se estudiaron retrospectivamente los antecedentes clínicos, el aspecto endoscópico de la mucosa gástrica y los informes del examen histológico de la misma en 100 niños (59 varones) de baja condición socioeconómica, de 2 a 17 (promedio 11) años de edad en que, por diferentes motivos, fue necesario realizar endoscopia digestiva alta para fines diagnósticos y se cotejaron con la presencia o ausencia de H. pylori registrada mediante prueba de la ureasa o en el examen histológico de los muestras obtenidas. Resultados: En 69 por ciento de los niños de la serie se identificó H. pylori, cuya prevalencia aumento con la edad (X2 11,97 p= 0,0025), llegando a 78 por ciento entre los 13 a 17 años. El agente se detectó en 30 por ciento de los niños sin lesiones histológicas y en 88 por ciento de los que tenían signos de gastritis. En 16 por ciento de los niños infectados no se registraban síntomas de la afección a pesar de estar contaminados y tener lesiones gástricas. Conclusión: estos resultados sugieren una fuerte asociación causa a efecto (X2 = 34,5, p= 0,00001) entre colonización de la mucosa gástrica con el agente y la existencia de signos endoscópicos e histológicos de gastritis

ABSTRACT

Gastrointestinal symptoms are common in acute falciparum malaria. Dyspepsia often occurs in such patients and sometimes it is exceptionally severe. However, the pathogenesis of the dyspeptic symptoms in malaria has not been clearly defined. Upper gastrointestinal endoscopy was performed in 40 patients with acute falciparum malaria in order to correlate the dyspeptic symptoms with the macroscopic (endoscopic) and microscopic (histologic) pathology of stomach and duodenum. The patients were divided into a dyspeptic group (n = 20, male/female ratio = 17/3, age range 18-50 years, mean age = 28.85 + 9.14 years), and a non-dyspeptic group (n = 20, male/female ratio = 16/4, age range 15-47, mean age 26.05 + 9.98 years). The findings revealed that dyspepsia correlated with topographic endoscopic pangastritis (p = 0.0014), the category of endoscopic antral gastritis (p = 0.013), and the histologic severity of antral gastritis (p = 0.0434). The results suggested that gastritis should be considered in acute falciparum malaria patients presenting with dyspepsia.
